The last sentence of the first paragraph of the "Output" section was in error. It should read:

In the state update, the Y vector is a vector indicating the speed at which key was pressed for each cue on the previous trial, regardless of its accuracy, regardless of its accuracy, and e_t is a binary scalar that is 1 if the previous response was an error and 0 if it wasn't.
